A Designation of Health Care Surrogate allows an individual the opportunity to maintain control over important medical decisions and appoint another person (known as the surrogate or agent) to make important health care decisions on his/her behalf in the event of incapacitation, such as his/her wishes on use of life prolonging procedures. This document is usually utilized in conjunction with the Living Will to ensure an individual’s decisions are honored. A Designation of Health Care Surrogate, Power of Attorney, and other advance planning may lessen or avoid the need for a guardianship on the principal’s incapacity.
